INSTRUCTIONS:

1. All blood draws are by appointment only.  No walk-ins are accepted.  
2. Per our agreement with Labcorp, we can not draw blood from any other healthcare providers or for non-patients.  
3. We charge a $20 administrative fee per appointment (due at the time of booking) which is used to pay for supplies and staffing. This fee is not reimbursable or run through insurance. 
4. If you do not get a confirmation email, it did not schedule properly.  You can use this confirmation email to reschedule or cancel.  Please do so as soon as possible to allow others to use your spot.  Since we have paid for a phlebotomist to be here, if you do not show up for your appointment, a refund will not be given.  If you arrive more than 5 minutes late, you may have to wait until the next available appointment.
5. As always, patients are free to go to any blood draw location, Labcorp, Quest, or other healthcare facility of their choice.
