INSTRUCTIONS:

1. All blood draws are by appointment only.  No walk-ins are accepted.  We normally have last-minute availability.
2. Per Labcorp, they will not draw blood from any other healthcare providers or for non-patients.  
3. We charge a $20 administrative fee per appointment (due at the time of booking). It is not reimbursable or run through insurance and is independent of your Labcorp bill. 
4. If you do not get a confirmation email, it did not schedule properly.  You can use this confirmation email to reschedule or cancel.  Please do so as soon as possible to allow others to use your spot.  If you do not show up for your appointment, a refund will not be given.  If you arrive more than 5 minutes late, you may have to wait until the next available appointment.
5. The Labcorp phlebotomist in our office is for our patients’ convenience and is not mandatory.  As always, you are free to go to any Labcorp, Quest, or other healthcare facility of your choice.
